If you have received assistance from The Sergey Foundation and would like to be listed as one of our Success Stories, please email us with the information requested below. Your story will be reviewed before being put on our website. We will inform you if/when your story gets posted.

*Please be as thorough as possible in explaining your story. The Sergey Foundation retains the right to edit, revise, reword, and otherwise alter your submitted story for publishing purposes. Our best editors are put in charge of this, and by submitting your story you acknowledge and accept The Sergey Foundation's right to editing. By submitting a photo, you also hereby acknowledge and provide The Sergey Foundation permission to use said photo on our website, in printing work, and for other purposes relating to fundraising and awareness efforts.
